Head injuries can happen in an instant. A fall from a ladder, a collision on the soccer field, a car accident, a slip on wet pavement. In those first critical minutes after impact, what you do or don't do can have a lasting impact on the outcome.

The challenge with first aid for head injuries is that severity is not always obvious. A person may appear completely alert and oriented after a serious head injury, only to deteriorate rapidly within hours. Conversely, a dramatic-looking scalp wound can bleed heavily while the underlying injury is relatively minor. The unpredictability is exactly why knowing how to respond correctly matters so much.

Step 1: Ensure Scene Safety Before You Approach

Your instinct when someone is hurt is to run toward them immediately. That instinct is understandable, but acting on it without pausing to assess the scene can turn one victim into two.

Before you take a single step toward the injured person, scan the environment for hazards. Is there ongoing traffic? Are there unstable surfaces, falling objects, or downed power lines nearby? If the injury occurred in a workplace, is there machinery still running or a chemical spill involved? If you are at a sports event, is the playing surface safe to enter?

Identify any hazards and determine whether they can be reasonably controlled. In some cases, you can redirect traffic, shut off a machine, or clear bystanders from the area. In others, the scene is genuinely unsafe and you should not approach at all. Calling 911 and waiting for professional responders who have the training and equipment to manage dangerous environments is not inaction. It is the right call.

If the scene is safe to enter, take a moment to protect yourself before making contact. If gloves or another barrier are available in a first aid kit nearby, use them. Scalp wounds bleed significantly, and protecting yourself from bloodborne pathogens is a standard precaution that should not be skipped.

As you approach, take note of how the injury occurred. This is called the mechanism of injury, and it matters enormously when assessing severity. A fall from standing height carries a different risk profile than a fall from a ladder, a diving accident, or a high-speed vehicle collision. The greater the force involved, the higher the likelihood of serious injury including potential spinal involvement. Step 2: Call for Help and Activate Emergency Services

Once you have confirmed the scene is safe, your next priority is making sure professional help is on the way. Do not delay this step while you assess the person in detail. In serious head injury situations, time matters.

Call 911 immediately if the person is unconscious, seizing, vomiting repeatedly, has a visible serious wound, or if the mechanism of injury involved significant force. Do not wait to see if they "seem okay." Head injuries can deteriorate rapidly, and emergency medical services need to be activated early.

If you are not alone, use bystanders strategically. Point to a specific individual and give a direct instruction: "You, in the blue jacket, call 911 right now and tell them we have a person with a head injury at this location." This specificity is important. When responsibility is not clearly assigned in a group, people often assume someone else has already handled it. This is known as the bystander effect, and it can cause dangerous delays. Naming someone directly eliminates that ambiguity.

When speaking to the dispatcher, provide your exact location first, then describe the nature of the injury and the current condition of the person. Stay on the line. Dispatchers are trained to guide you through what to do next while help is in transit. They can tell you whether to start CPR, how to position the person, and what information to gather. Do not hang up until they tell you to.

Even if the injury appears minor at first glance, head injuries warrant medical evaluation. A person who seems alert and oriented after a bump to the head may still have a concussion or, in rarer cases, a more serious injury with a delayed onset of symptoms. When in doubt, err on the side of caution and encourage the person to be evaluated by a medical professional.

Step 3: Assess Responsiveness and Visible Symptoms

With help activated, turn your full attention to the injured person. Your goal here is to get a clear picture of their level of consciousness and visible signs of injury without moving them unnecessarily.

Start by checking responsiveness. Tap their shoulders firmly and ask loudly, "Can you hear me? Are you okay?" If they do not respond, check for normal breathing. If the person is unresponsive and not breathing normally, begin CPR immediately if you are trained, and use an AED if one is available. Head trauma can result in cardiac complications, and CPR is the appropriate response when a person is unresponsive and not breathing. Do not hesitate out of concern about the head injury itself.

If the person is responsive, keep them as still as possible while you assess. Ask simple orientation questions: What is your name? Do you know where you are? Do you remember what happened? Confusion, disorientation, or the inability to recall the event are significant warning signs.

Look carefully for visible signs of serious injury. Bleeding from the scalp is common and can look alarming, but also watch for these specific signs that suggest a more serious injury:

Bruising behind the ears (Battle's sign): This can indicate a skull fracture at the base of the skull. It typically appears hours after the injury rather than immediately.

Bruising around both eyes (raccoon eyes): Another potential indicator of a basilar skull fracture, also often appearing with a delay.
